service at no extra charge.   Please bring this form in to be filled out completely.  

Send your sizes at least 2 weeks prior to the wearing date to avoid a late-order fee of $25.  Sizes submitted less than one week
before the pick up date may be charged more.

We cannot be held responsible for measurements not performed in our store.  You MUST try on your tuxedo in a timely manner.  If
your tuxedo does not fit  we will do everything possible to make any necessary corrections, however, any special freight charges will
be at your cost.
